Terms and conditions for Unlimited Monthly Membership

  • FIT4MOM BIRMINGHAM MONTHLY MEMBERSHIP AGREEMENT
    This contract is made between the client and FIT4MOM BIRMINGHAM and is a month-to-month agreement (3 month initial commitment) and except as provided in the initial 5-day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend unlimited and all programmed classes.


    AUTHORIZATION FOR PAYMENT
    I authorize FIT4MOM Birmingham to initiate a charge to the account entered into Pike 13 for my dues. This authorization is to remain effective until I have canceled my membership in writing through the FIT4MOM Birmingham administrator.

    I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Birmingham to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend classes who have unpaid balances. If a membership remains unpaid it will be canceled.

    CANCELLATION POLICY

    Monthly Memberships may be canceled (without an early cancellation fee) so long as the Member has fulfilled their initial membership commitment and provides at least 14 days notice prior to their next scheduled billing date. Any cancellation of a pass prior to the initial commitment timeframe will result in a $60 Early Cancellation Fee. We regret there are no exceptions. No refunds will be given for dues once it has been paid. ALL Membership Packages automatically renew on the first of the month. In order to cancel a membership, the Member must notify FIT4MOM Birmingham via email to birmingham@fit4mom.com. A confirmation email acknowledging receipt of this form will be emailed to you within 48hrs. Your notice period will start from the day the form is submitted. Once a membership is canceled, if the member wants to re-enroll FIT4MOM Birmingham will waive the sign up fee if membership is reactivated within 3 months of cancelation. After 3 months of cancellation, a full sign up fee will be charged to reactivate membership. Instructors are not authorized to cancel memberships.


    HOLD POLICY

    Members may place their membership on hold for reasons such as medical conditions and maternity leave. In order to place a hold on a membership, the Member must notify FIT4MOM Birmingham via email to birmingham@fit4mom.com. Members must notify FIT4MOM Birmingham 14 days prior to the desired hold start date. A confirmation email acknowledging receipt of the request will be emailed to you within 48hrs. You notice period will start from the day the form is submitted. Memberships can be placed on hold for no less than 30 days and no longer than 3 months. The minimum commitment (3-months) on your plan must have been completed before a hold can be requested. Hold starts will coincide with the next billing date and can end on any day of the month. When a hold period is ended, the monthly auto payments will automatically resume on the return date. Once the end of the hold period has been reached, the Member’s original Membership contract will resume automatically. It is the members' responsibility to keep track of their return date. Instructors are not authorized to place a hold on memberships.

    FIT4MOM OnDemand Monthly Subscription
    With your paid monthly membership you are given free access to FIT4MOM OnDemand. You will have unlimited access to OnDemand workouts. When your FIT4MOM membership is properly canceled according to the Terms and Conditions, your OnDemand subscription will be automatically canceled. If you would like to keep your OnDemand subscription you can re-enroll using the same login information and purchase your desired paid subscription.

    INITIAL 5-DAY CANCELLATION POLICY

    You, the buyer, may cancel this agreement at any time prior to midnight of the fifth (5th) calendar day after your initial payment transaction. To cancel this agreement written notice must be received by FIT4MOM Birmingham within the initial five days. You are responsible for paying for the classes already attended at the drop-in rate.
